The following items are included in our Registration Packet and all forms must be completed prior to your child starting school:

    2. Application Information for 2017-18
    3. Preschool Contract Registration form for 17-18
    4. ID and Emergency Information
    5. Pre-admission Health History
    6. Physician’s Report
    7. Notification of Parent’s Rights
    8. Notification of Personal Rights
    9. Field Trip Waiver
    10. Consent for Medical Treatment TWO ORIGINAL COPIES REQUIRED: one for school files, the other accompanies students on all field trips
    11. Photo Release Form
    12. Parent-Support-Jobs 17-18

Level 1 Parent (working in classroom) Requirements:

  • TB Test
  • Vaccine proof of pertussis (the P in DPT) & measles (the 1st M in MMR)
  • Annual “flu shot” between August 1 – December 1

Level 1 and 2 Parent Requirements:

  • Copy of Drivers License
  • Copy of Current Auto Insurance
